Join a multidisciplinary team from Whidbey Health to learn more about your options for Advance Care Planning. Advance Care Planning (ACP) is a way for you to express their goals of care anywhere along the health care continuum.
We will begin with a short PowerPoint presentation and a Q & A session and conclude with the opportunity to talk one-on-one with a social worker, provider, or registered nurse (RN). These are informational sessions only; no medical care will be provided.
